- DOT Medical Exam Report (Fill out pages 1 & 2 to review with Certified Medical Examiner).
What to expect at your appointment:
- Review of health history
- Vital Signs including blood pressure and heart rate
- Vision and hearing tests
- Height and weight measurements
- Comprehensive urinalysis
- Sleep apnea screening
- Cardiovascular risk assessment
At the end of your visit you will be provided with a
Federally issued Medical Exam Certificate to take to the DMV.
